Collect practical evidence
Useful evidence can include support logs, reports, therapy recommendations, risk notes, changed circumstances and examples of what happens when support is not available.
Connect support to goals
Explain how each support helps with daily living, community participation, independence, health, safety or skill-building.

Changes in home life, health, behaviour, work, school, family support or living arrangements can all affect support needs.

A provider can share service information and help explain current supports, but formal plan advice may also involve a support coordinator or allied health professional.
